Definition: Benefit plan and employee information that is incorrect, but is used anyway either 1) Without the knowledge of the user, or 2) By design (in a rush to get the forms out the door).
Missing Form Recipients
Definition: Active or inactive employees that should have received a form because they were ACA eligible (the penalty is $250 for each missed recipient).
Missing Controlled Groups
Definition: A group (regardless of size) that is considered one employer vis-à-vis a parent-subsidy, brother-sister or any combination of these regardless of the legal business formation type.
Wrong Premium Amounts
Definition: Part II. Line 15 lowest monthly premium for single only coverage that is incorrect regardless of reason (e.g. bad data, wrong amount) for either all 12 months or any month(s).
Wrong Employee Status Codes
Definition: Part II. Line 16 codes that are incorrect regardless of reason (e.g. bad data, inaccurate safe harbor method, flawed programming) for either all 12 months or any month(s).
Definition: Any combination of spouse, domestic partner or child(ren) that were inadvertently either not included or wrongly assigned to a recipient that has no relationship with the respective dependent.
